Join Prisma Health and My Life My Choices on Saturday, March 21 at 10 a.m. in Columbia for a free community Advance Care Planning symposium.

Prisma Health Homecare and Hospice are offering this free advance care planning symposium, open to all community members in South Carolina.

Educational information and refreshments will be provided. Participants will learn to take part and be in charge of their health care plan. Having these conversations will allow adults to understand possible future health care choices and think about what choices reflect their values and beliefs.
