April is South Carolina Healthcare Decisions Month!

This April, we are urging all of South Carolina to participate in Healthcare Decisions Month:

Our collective effort to highlight the importance of advance care planning, and to provide tools for making these decisions.

It is often the case that advance care planning occurs in a clinical setting. But engagement should expand beyond the healthcare system, however, to meet people where they live, work, and pray.

What matters most to you? What are your wishes for the future, and what you would want if you became seriously ill?

Use our ACP Bubble Map to answer these questions, and then pick up the phone, start a video chat or send an email—have an open, honest conversation with someone important about #WhatMattersMost.

There is perhaps no more appropriate time for us to think and talk about these questions than while confronting the realities of a global pandemic.

We are challenging healthcare facilities, healthcare professionals, religious leaders, the legal community, and others across the state to use this month to drive the conversation and lead by example.

This important month exists to inspire, educate, and empower each of us to:

  • THINK about your values, beliefs, and goals… How do they relate to your healthcare and quality of life?

  • TALK with loved ones openly and honestly—before a medical crisis happens—so others know what matters most to you at the end of your life.

  • WRITE IT DOWN in the proper South Carolina Advance Directive form and share it with your loved ones and your doctors.

My Life My Choices seeks to promote advance care planning by empowering all South Carolinians to make these critical decisions.

We work with healthcare providers to increase awareness about Advance Care Planning, and provide resources to help you make informed decisions about end-of-life care and ensure they are honored.
